One Chart | Patient is a secure online portal that serves as your most important source of medical information. This portal can be accessed from your computer or smartphone and through it you can:

  • Ask your doctor questions
  • Access parts of your medical record (e.g., immunizations)
  • Pay bills and view billing statements
  • Request return appointments
  • Request prescription renewals
  • View test results

How to Sign Up

You will automatically receive an invitation by email or text to sign up for One Chart after you schedule your first appointment. You can activate your account before, during or after your appointment. Only students who are 19 or older or are 18 years old with a Power of Attorney form on file at the health center will receive and invitation to sign up for the portal. Due to Nebraska's age of majority law, if you are 17 or younger or are 18 without a Power of Attorney form on file, you will be ineligible to sign up.

One Chart | Patient is a secure, online web portal that offers personalized access to parts of your medical record. While signing up is not required, we think you will appreciate the benefits and convenience of our online patient portal.

Does it cost anything?

There is no cost.

How do I enroll?

You will be provided an activation code at clinic appointments, or you can request one from our website, www.onechartpatient.com. You will be given a one time activation code and then prompted to create a login Username and Password. As a One Chart | Patient user, you are responsible for maintaining security of your Username and Password from unauthorized individuals.

What features are available?

One Chart | Patient is a convenient way to:

  • Request/Cancel appointments
  • Pay bills
  • Ask your health care team questions
  • Access a range of your health care information
  • Most lab test results are available two business days after completed
  • Most procedures results are available 30 days after completed
  • Request updates to your list of medications, health problems, or allergies
  • Request prescription renewals
Will I be able to see the same information my provider sees?

You will be able to see much of the same information that your provider views. Some sensitive results are released at the discretion of your provider. If you do not see your results in One Chart | Patient, please call or message the ordering provider's office.

How do I communicate my medical concerns?

One Chart | Patient offers a messaging service with your health care team. You are able to communicate with providers that you have had an appointment with in the past two years or in the upcoming six months. We ask that you do not use One Chart | Patient to communicate serious medical concerns with your health care team. Please schedule an appointment with your provider to discuss immediate medical concerns or problems. If you are experiencing a medical emergency, call 911 immediately.

Is One Chart | Patient a secure site?

Yes. Your privacy is important to us. This website is designed to meet federal laws involving the privacy and security of your personal information. All information sent is encrypted to ensure your privacy is maintained. Additionally, the information shared is protected by layers of security within our infrastructure to further safeguard your information.

Is there an app available for my mobile phone or tablet?

Yes. Search and download MyChart in the Apple Store (Apple) or Google Play (Android). Once downloaded, open MyChart and select Nebraska Medicine as your provider. You must set up a One Chart | Patient account before using the application.
